  2. Copy of City of Owensboro Business License
  3. Certificate of Insurance with City of Owensboro listed as additional insured
    • Death or injury to any one person $25,000
    • Total liability for death or injury to person $50,000
    • Property damage $10,000
  4. Copy of Green River District Health Department approval
  5. Photo of Vehicle
  6. Proof of Vehicle Registration
  7. Proof of Vehicle Insurance
  8. $250 for City of Owensboro(excludes Downtown Entertainment District) or $650 for City wide(Includes Downtown Entertainment District)
